Reporting Documents

Grants are awarded annually for the period April 1 to March 31.  Organizations that receive an award are required to sign and return the funding agreement within 14 days of notice of the award and are also required to provide periodic program reports on August 15, December 15 and April 15, or next business day.  

Impact Grant Reports are to be submitted electronically to Karen Hemberger, Vice President of Impact, k.hemberger@unitedwaymadisonco.org.

Mini Grant Reports are to be submitted electronically to Kim Rogers-Hatfield, Vice President of Operations, k.hatfield@unitedwaymadisonco.org.
