We need your input! Complete a tobacco/alcohol survey
Healthy Tobacco Free Madison County is collecting data regarding use of tobacco/alcohol/other drugs in Madison County. This information will be used to assist social service organizations in the development of and support for programs that positively impact the health of Madison County individuals and families.
To facilite information gathering from a wide population, the survey has been made available online and can be accessed at http://www.questionpro.com/akira/TakeSurvey?id=1149981. The survey will be collected through March 15.
