Healthy, Tobacco Free Madison County

The following survey is being collected as part of a Madison County substance use profile.  This profile is used to inform public policy and apply for grants that support treatment, law enforcement, and/or prevention efforts in our county.  All responses are anonymous.  Must be 18 or older to participate.  The survey takes about 3 min. and your participation would be greatly appreciated.  Thank you.
